On this page we have published information about how to register with us, MSO and MDSO contact forms and an introduction to National Patient Safety Alerts. Please note that If you are a registered NHS user with login access, additional user guides and helpsheets are available once logged in. 

Registration

If you would like to register to receive alerts and safety messages then please contact us at safetyalerts@mhra.gov.uk and provide the following information:

• The email address you would like alerts and safety messages to be sent to
• The full name of your practice / organisation and your NHS ODS code if you have one
• The full address including postcode

MDSO and MSO forms

• An updated Medical Device Safety Officer (MDSO) form can be found at this link.
• An updated Medication Safety Officer (MSO) form can be found at this link.
